Nurse
Jessica Soliz, RN
Hello Mustangs! I am excited to be joining Dahlstrom Middle School as your school nurse. This will be my 5th year as a school nurse for HAYS CISD.
My role as the school nurse is to support our students' education by promoting and maintaining good health, as well as taking care of acute injury and illness when it occurs.
School nursing is a professional registered nursing specialty that involves identification, assessment, prevention, and intervention to remedy or modify students' health needs. The nurse's goal is to promote the optimum health of students and the students' abilities to achieve their individual educational goals.
Nurses monitor required immunizations, perform mandated health screenings, track communicable diseases, plan for emergencies and provide direct care to students. Please click here to learn more about Hays CISD District Health Services.
We would like to assure you that medical information about your child is treated as confidential by the nurses and all school staff. We do not share information about your child with outside parties nor can we discuss health concerns of any other student with you.

Medications at School
Medication reminder: Students in middle school are ONLY allowed to self-carry inhalers and epi-pens (with physician approval). Please ensure we have the appropriate authorizations on file for your student to self-carry these medications if needed. All other medications must be kept in the nurse's office with the appropriate authorization on file. Medication Authorizations and Action Plans can be found here:
Immunizations
All students must be up to date on vaccines or have a current, valid exemption form on file whether attending virtually or in person for the school year. Please visit here for a full list of Texas minimum vaccine requirements for school. Updated immunization records may be emailed to jessica.soliz@hayscisd.net. Exemption forms for immunizations can be requested here. If your student receives immunizations throughout the school year, please forward updated immunization records to the nurse at jessica.soliz@hayscisd.net. Need help getting vaccines? Click HERE for community resources for immunizations.
*Reminder: 6th grade students will need proof of Tdap and MCV before starting 7th grade.

Health Screenings
Texas law requires schools to conduct hearing, vision, spinal, and Acanthosis Nigricans (AN) screenings to help identify potential health concerns early. New legislation now requires parent to opt in for these screenings. Required grades for Vision and Hearing screening are: PK, K, 1, 3, 5, and 7. Required grades for Spinal screenings are: 5th and 7th grade girls and 8th grade boys.
In addition to these required grades, all new to district students are required to be screened for vision and hearing regardless of grade. Some students may need to be screened during the year for special programs like special education as well as a concern for vision or hearing in the classroom. Parents will be notified of any screenings that are not passed for follow up care referrals.
Please email me to jessica.soliz@hayscisd.net if you have questions or your child loses/breaks their glasses and you need help getting a new exam/pair.
Affordable health care - if your child doesn't have medical insurance or is about to lose it, I do have resources to help get them care when they need it.
Clothing Donations Needed
Accidents happen regardless of age that require a change of clothes. Spilled food, nosebleeds and unable to make it to the bathroom in time are just a few of the reasons our students need an extra outfit. We try to have a variety of sizes on hand to help get students back to class. Our Clothing Closet in the clinic is always in need of gently used pants/shorts. Elastic waist clothing is preferred as it fits a variety of sizes and is free of snaps/buttons/zippers which can be difficult for some students. Donations are always welcome.
