From the Nurse - All students are required, by the state of Texas, to be current on their immunizations or provide a current original exemption affidavit to remain in school. All incoming 7th graders must have documentation of having the following immunizations: 1. Tdap within the last 5 years, and 2. A meningococcal immunization. For your student to receive a schedule for the 2026-2027 school year, all updates must be on file by the first day of school. They will NOT be able to attend school until these records are up to date. Please be proactive and take your child for their required immunizations. Please visit Nurse's Notes/Health Information on our Chapa website, under Parents and Students. Any new health-related information will be posted prior to the start of the school year. You can drop the records off at school or send them via email to Betzabe Britton at Betzabe.Britton@hayscisd.net.
Athletic Physical Information for 7th and 8th Grade - Any student wishing to participate in athletics and/or cheer at any Hays CISD middle or high school must obtain an athletic physical before the start of the school year and submit it AT THE LATEST by August 13th to remain in the Athletic class. All physicals should be submitted as soon as possible, as students will not be able to participate until a physical is on file. It is preferred for all students to turn in BEFORE THE LAST DAY OF THE SCHOOL YEAR. The physical must be dated on or after April 1. ALL RANK ONE forms must be completed and signed electronically by a legal parent/guardian. https://hayscisd.rankone.com/Pro/Login.aspx. All incoming freshmen will turn physicals into Lehman High School Athletic Trainer, Rikkie Fernandez. All incoming 7th- and 8th-grade athletes will submit hard copies of their physicals to Coach Rodriguez and Coach Sakewitz at Chapa. This applies to all sports, spring and fall. Hays CISD, following the University Interscholastic League, requires a current athletic physical to be completed before participating in athletic practices, classes, or competitions, both in-season and off-season. All athletic physicals must be completed on the 2026-2027 physical form. No other form or note will be accepted for participation in athletic activities. You may pick up a copy of this physical form at any middle school or high school or please check our website. Students will be removed from athletics if they do not get their physical.
