• Hays CISD is a workers’ compensation insurance subscriber and this coverage is provided at no cost to employees.
  
  
• Any employee injured on the job must submit a report of their injury to his or her supervisor within 24 hours of the injury or when the employee first knew the injury or illness might be work related.
  
  
• Supervisors must conduct and document an accident investigation within 48 hours of the event.

Document an Accident Investigation:

Supervisor print form D13-B, complete it, print it, sign it and scan to Margaret.Martinez@hayscisd.net or fax to (512) 268-7384.

Witness Statement:

Print form D13-D, have all adult witnesses complete the form and scan to Margaret.Martinez@hayscisd.net or fax to (512) 268-7384.
