It is important that parent(s)/guardian(s) of students with seizures communicate fully with the school nurse about the student’s needs. If possible, plan to meet with the nurse prior to the first day of attendance. Students with seizures can attend school, but school nurses need to know some information to keep the student safe at school. 

Hays CISD requires the following forms be signed by the parent/guardian for all students seeking diabetes care while at school: 

1. Medication Authorization (English)
2. Medication Authorization (Spanish)
3. Seizure Action Plan (English/Spanish)
4. Seizure Questionnaire (English/Spanish)
