  • As we come to the end of the 2022-23 school year, congratulations to our graduates.  I wish them much success in their future endeavors. 

    A reminder: for returning LOA and new LOA students in the 23-2024 school year. Parents/guardians- the following MUST be completed:

    1. Immunizations (see below) or Reason of Conscience
    2. Emergency Health Card
    3. Doctor's orders for medications-long term or self-carry; ESPECIALLY Asthma inhalers
  •   The current information for HAYS CISD COVID guidelines can be found on the HAYSCISD district website- (click the big coronovirus screen) and The Center for Disease Control- www.cdc.gov.
     
    Please remember to screen your student daily before sending them to school.  Always keep them home if they are not feeling well.  Not just for COVID symptoms.  Nausea, diarrhea, severe headache, temperature greater than 99 degrees are reasons to NOT send your child to school.  Please consider that they may have the flu.  If possible, get them tested.  
     
         If your child has any ONE of the following symptoms, is lab-confirmed with COVID-19 OR has been home tested for COVID-19 and is awaiting results, DO NOT SEND THEM TO SCHOOL.  Please contact me.  
            
               SYMPTOMS:
    • new loss of taste or smell
    • new onset of fatigue, muscle or body aches
    • Temperature of 100 degrees Fahrenheit or higher when taken by mouth
    • sore throat
    • diarrhea, nausea, vomiting or abdominal pain
    • new onset of headache, especially with a fever
    • new uncontrolled cough that causes difficulty breathing or for students with a chronic condition such as asthma or allergic cough-a change from their baseline
     If anyone in your home is awaiting the results of a COVID test-DO NOT send your student to school and notify the school nurse.
    If your student has been exposed to COVID-19 at home or elsewhere, keep them home and notify the school nurse.
     
    It is requested that positive test results for COVID or Influenza be sent to the nurse.  joan.weston@hayscisd.net
     
     
    IF YOUR STUDENT HAS BEEN OUT OF THE  UNITED STATES,  FOR MORE THAN 3 WEEKS-HE/SHE IS REQUIRED TO HAVE A TUBERCULOSIS TEST. (THIS DEPENDS ON THE COUNTRY VISITED). Please contact me for further information.  
     
     
     
     
     
     
     

     

  • IMMUNIZATIONS

    • All students must be up to date on vaccines or have a CURRENT, valid exemption form on file whether attending virtually or in person for the school year.  This information may be found by using the IMMUNIZATION TAB   (on the left of this screen)
    • Updated immunization records can be emailed to Joan.weston@hayscisd.net
    • Exemption forms for immunizations may be found on the  IMMUNIZATION TAB

    COLLEGE REQUIREMENTS: All students entering an institution of higher education are required to show proof on an initial meningococcal vaccination or a booster dose during the five year period prior to enrolling.  The student must get the vaccine at least 10 days before the semester begins. See IMMUNIZATION TAB

  • #1.  If you have not completed an EMERGENCY HEALTH CARD for your student for this school year, please do so.  This allows us to treat your student in the nurse's office, give medicine and transport in case of an emergency situation.  If you need the EMERGENCY HEALTH CARD form-please email me joan.weston@hayscisd.net and I will be happy to send you either English or Spanish version. 

    #2.  If your student requires ANY MEDICATION while on campus-whether it is a daily scheduled dose or, as needed medication such as asthma medication or short-term pain medication, there must be a signed doctor's order.  These blank forms may be found to the left of the screen.  Please notify me of any medication your student may be taking.

    #3.  Please notify the LOA nurse of any condition regarding your student which you feel is relevant.

     

    I would like to assure you that medical information about your child is treated as confidential by the nurse and all school staff.  We do not share information about your child with outside parties nor can we discuss health concerns of any other student with you. 

  • Please be aware that if your student is ill, they must come see the nurse before they can be released to go home.  If the student is contacting parent/guardian directly without visiting the nurse, it is considered an unexcused absence.
