Report an Absence

  • Documentation needs to be submitted within 5 days of the absence. The note should include: Student’s full name, ID #, Date(s) of absence, Reason for absence, Parent contact phone number, Parent signature (or electronic signature if sent by email). We DO NOT accept phone calls or voicemail messages as documentation for a student's absence.

    Please use the link below to report a student absence.  After submission the form will be automatically routed to the HHS Attendance Office (  You will have the option to print the form after submission.
