Johnson High School Sports Medicine
The Johnson High School Sports Medicine Program is very important to the success of the Jaguar athletic programs. We provide injury prevention, care, treatment, and rehabilitation for our student athletes.
*NOTICE REGARDING PHYSICAL EXAMINATION REQUIREMENT* (For 2020-2021 School year only)
In light of the many challenges caused by the COVID-19 pandemic, the UIL has waived certain requirements for students to undergo a physical examination prior to participation in UIL marching band or UIL athletic activities.
Athletes with current physicals for 2019-2020 School Year
- Students who have previously submitted to the District a completed current Pre-Participation Physical Evaluation (PPE) form will not be required to submit a new PPE form in order to be eligible to participate in the 2020-2021 school year unless the student answers “Yes” to questions 1-6 on the Medical History Questionnaire. All students will be required to complete the Medical History Questionnaire prior to participation in 2020-2021.
Students New to Athletics/Incoming 7th Grade Athletes & 9th Grade Band Students
- Students seeking to participate in UIL marching band or UIL athletic activities for the first time and who do not already have a completed PPE form on file with the District will still be required to complete and submit a PPE form prior to participation
Online Form & UIL Physical Form Links *All Athletes and their Parents/Guardian must complete the RankOne forms by reading and signing electronically* Forms need to be signed digitally with a full signature. Initials will not be accepted.
The best way to submit Physicals Right now is to email them to either of the Athletic Trainers, since we don't have a way to collect paper physicals at this point. (Will update this later)
